The Sanskrit word Kativasti combines "kati" (lower back, lumbar region) and "vasti" (reservoir, container). The therapy is described in detail in the Sushruta Samhita and Sahasrayoga as a localised Sneha (oleation) treatment for Vata-pattern lower back disorders — chronic pain, stiffness, sciatica, and degenerative spine conditions.

The mechanics are simple but precise. A ring is built around the lower spine using kneaded wheat-flour dough — typically about 4 inches tall and 6 inches in diameter. Warm medicated oil is poured into the reservoir to fill it. The oil is held at a controlled temperature for the entire 30 to 45-minute session, with the therapist replacing the cooled oil with fresh warm oil approximately every 10 minutes.

The therapy works through three mechanisms. First, sustained warmth penetrates deeper into the lumbar tissues than any handheld massage technique. Second, the medicated oil — selected for your specific condition — is absorbed directly through the skin into the lumbar fascia, paraspinal muscles, and surrounding connective tissue. Third, the prolonged contact time allows for therapeutic effects that quick massage cannot achieve.

THE TREATMENT PROTOCOL

0-10 min

Phase 1 — Assessment & Preparation

The session begins with a brief clinical reassessment — current pain level on a 0-10 scale, range of motion in lumbar flexion and extension, palpation of the paraspinal muscles. The therapist marks the precise area to be treated, then shapes the dough ring directly on your lower back using kneaded whole wheat flour. The ring is sealed to the skin with light pressure to ensure no oil leakage. The medicated oil is warmed to approximately 40-42°C in a brass vessel.

10-15 min

Phase 2 — Initial Oil Pour & Settling

Warm medicated oil is gently poured into the dough reservoir until it fills to about 2 inches deep. You will feel immediate warmth penetrating the lumbar region. The first 5 minutes are about the body settling into the warmth — most patients describe immediate softening of the surrounding musculature. The therapist remains present throughout, monitoring oil temperature with a small ladle.

15-50 min

Phase 3 — The Main Treatment

The oil remains in the reservoir for 30-35 minutes of sustained therapy. Every 8-10 minutes, the therapist removes the cooled oil with a small ladle and replaces it with fresh warm oil, maintaining the temperature window throughout. You remain still on the droni for the entire phase. Most patients describe the sensation as deep penetrating warmth rather than discomfort. This is the working phase of the therapy — the medicated oil components are absorbing into the lumbar tissues.

50-60 min

Phase 4 — Removal & Recovery

The dough ring is carefully removed, residual oil is wiped (but not washed off), and a gentle 5-minute massage of the lumbar region completes the session. You are asked to remain on the droni for an additional 5 minutes. The oil film left on the skin continues to absorb for several hours. We recommend not showering for at least 3 hours and avoiding cold environments (air conditioning, swimming pools) for the rest of the day.

Followed where indicated by Patra Pinda Sweda (medicated leaf bolus therapy) or other adjunct treatments. Total clinic time approximately 75-90 minutes including changing, brief consultation, and post-session rest.
